Method for anterior abdominal wall plastics in umbilical hernias with diastasis of rectus abdominis muscles

Background. Evaluation of surgical treatment’s results of umbilical hernias in combination with diastasis of the rectus abdominis muscles according to the original technique. Methods and materials. In the period from 2015 to 2018, patients with umbilical hernias in combination with diastasis of the rectus abdominis muscles, paraumbilical hernias, and epigastric hernias were operated on. Among these patients, patients were distinguished who were operated on the onlay method and according to the original method, it consisted in the application of the mesh prosthesis onlay in the manner without fixing the mesh prosthesis to the white line of the abdomen, which reduces trauma to the white line, the risk of hernial protrusions at the border of the “mesh prosthesis – tissue”. Results. In the postoperative period (n=73), early complications were studied, including seromas, infection, hematomas, postoperative scar infiltrate, and long-term results were studied in 48 patients. According to the results of examination of patients and questioning in patients operated according to the original method (n=33), there were no recurrences of the disease after 3 years, in contrast to patients operated on in the traditional way, where relapse of the disease was observed in 6 %. Conclusion. The presented results allow us to consider modified anterior abdominal wall plasty in diastasis of the rectus abdominis muscles in combination with umbilical, paraumbilical hernias and hernias of the white line of the abdomen as reasonable and effective.
